When a single parent is applying passport for his/her child without getting the consent of his/her spouse, then Annexure C has to be enclosed along with the other documents to process the passport application for the minor.
When the following situation satisfied, then Annexure C can be enclosed.
- If you are a single parent, either the father or the mother of the minor child,wishes to apply for minor passport for your child
- If the minor’s other parent does not give consent to apply or sign in for the passport
Declaration of Parent/Guardian for Minor Passports -Annexure D
Annexure D is an affidavit that is enclosed by the parents or the legal guardians when applying for a minor passport. By this affidavit, they confirm that they take sole responsibility for the minor’s expenditure and all the information & details provided by them is true. Standard General Mandatory Affidavit- Annexure E
Annexure E is a general mandatory affidavit given by any Indian citizen who wishes to apply for an Indian passport. Annexure E can also be said as a self-declaratory affidavit which affirms that the applicant’s basic information, citizenship, and all other details are to be true. Apart from this, the applicant has to declare that he/she has no criminal record in the preceding five years. Hence Annexure E also serves as No Criminal Record Affidavit.
Who should Make Annexure E?
Any Indian citizen who has to declare their address, personal information, any criminal background and citizenship can enclose Annexure E when applying for a passport.
